About the Role:
- Part-time position in Princeton and Sullivan, IN area with a pay rate of $50/hour.
- Collaborate with patients and multidisciplinary teams to develop personalized treatment plans using evidence-based practices.
- Conduct assessments, diagnoses, treatments, and maintain accurate medical records.
- Create weekly rounding schedules for assigned facilities.

Requirements:
- Certified Nurse Practitioner license
- Preferred 2+ years in Skilled Nursing facilities
- Strong communication, organizational, and time management skills
- Ability to travel within a 12-20 mile radius
