Overview
Seeking an Advanced Practice Provider to deliver outpatient primary care at a Family Care Clinic in Prineville, OR.
They will evaluate, diagnose, treat, and document patient encounters across all ages, coordinate services within a Medical Home model, and develop comprehensive treatment plans incorporating behavioral health, pharmacy, and other specialties.
The role involves working at least 40 hours weekly with scheduled patient contact, participating in call rotations, and collaborating with a multidisciplinary team supported by behavioral health, pharmacy, and care management.
Key duties include following evidence-based family medicine practices, prescribing medications, interpreting diagnostics, providing patient education, and maintaining documentation per standards.
Qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licensure, certifications, and outpatient care experience. Preferred candidates have 2+ years in family medicine, familiarity with Medical Home models, and Epic EHR experience.
Compensation ranges from $154,568 to $165,351 annually, with benefits such as health insurance, retirement plans, and paid time off. Relocation to Prineville is required.
