RN Care Manager (Emergency Room)


Coordinate nursing care for high-acuity, diverse patients in a fast-paced, safety-net setting. Perform comprehensive assessments and develop, implement, and refine patient-centered care plans using evidence-based practices to improve outcomes and reduce avoidable readmissions.


Key duties

  • Assess patient needs and determine appropriate care interventions.
  • Plan and coordinate care across clinical and community resources.
  • Support safe transitions of care and follow-up continuity.
  • Collaborate with physicians, social workers, and community partners.
  • Educate patients and families and advocate for needed resources.
  • Contribute to equitable, compassionate care aligned with organizational mission.

Qualifications & skills

  • Current RN licensure and strong emergency/acute care knowledge.
  • Strong clinical assessment, care planning, and care coordination skills.
  • Ability to manage complex cases and work effectively in a high-volume environment.
  • Excellent communication, education, and advocacy skills.
  • Team-oriented mindset with experience partnering across disciplines and services.
