Licensed Practical Nurse (LPN) – Home Health
Provide compassionate skilled nursing care to pediatric and adult patients in home and community settings.
- Administer medications and perform prescribed treatments.
- Provide wound care and other required clinical services.
- Monitor vital signs and observe patient clinical status.
- Follow individualized care plans and ensure care is delivered according to instructions.
- Document patient care and assessments in electronic health records.
- Communicate patient changes and updates to registered nurses and physicians.
Qualifications
- Active LPN licensure.
- Experience or ability to deliver skilled nursing services, including medication administration and wound care.
- Strong clinical observation, documentation, and communication skills.
- Comfort working independently in patient homes while collaborating with the care team.
