Overview
Licensed Practical Nurse (Residential Living Center)
Provide compassionate, hands-on nursing care to residents in a residential setting. Administer medications, monitor vital signs, perform basic treatments, and document patient care in the EHR. Work as part of a collaborative, patient-centered team to support individualized care plans and ensure continuity of care.
Key Duties
- Administer prescribed medications safely and accurately
- Monitor and record vital signs and resident responses
- Deliver basic treatments and required nursing interventions
- Document assessments, care provided, and changes in condition in the EHR
- Communicate clinical changes promptly to the care team
- Support resident and family education as appropriate
Qualifications & Skills
- Active Licensed Practical Nurse licensure
- Strong clinical assessment and medication administration skills
- Proficiency with EHR documentation
- Compassionate, patient-focused approach and effective communication
- Ability to work collaboratively within a healthcare team
