Overview
RN Care Manager provides inpatient care coordination and supports safe transitions across the care continuum.
Key Responsibilities
- Conduct comprehensive nursing assessments and develop patient-centered care plans.
- Collaborate with physicians, therapists, social workers, and other care team members to improve clinical and financial outcomes.
- Lead discharge planning, including identifying and coordinating appropriate post-acute services.
- Educate patients and families on care needs and transition plans.
- Ensure accurate, thorough documentation in the EHR.
Qualifications & Skills
- Registered Nurse with strong assessment and care planning skills.
- Excellent communication and interdisciplinary collaboration skills.
- Proficiency in discharge planning and patient/family education.
- Strong attention to detail for documentation and care coordination.
