PRN LTC LPN (Long-Term Care)
Provide compassionate, skilled nursing care to patients in long-term care settings. Deliver high-quality support while contributing to positive patient outcomes in a team-focused healthcare environment.
- Assess patient needs and deliver appropriate nursing interventions
- Administer medications and follow established care plans
- Monitor patient condition and report changes to the care team
- Maintain accurate, timely documentation of patient care
- Support safe, respectful patient care and uphold facility standards
- Collaborate with physicians, RNs, and other healthcare professionals
Qualifications & Skills
- Active Licensed Practical Nurse (LPN) license
- Experience or strong knowledge of long-term care practices
- Compassionate approach and strong patient-focused communication
- Ability to manage clinical responsibilities with sound judgment
- Reliable documentation and attention to safety protocols
